1. Consent & Acknowledgment I, the undersigned parent or legal guardian, hereby give consent for my child (the “Minor”) to participate in fitness training, classes, and related activities provided by LÊ TONE STUDIO (the “Company”), including but not limited to exercise sessions, use of equipment, and participation in any on-site or off-site activities. I understand that participation in physical training carries inherent risks of injury, which may include but are not limited to: muscle strains, sprains, broken bones, heat exhaustion, cardiovascular complications, or other injuries that may occur during physical activity. 2. Assumption of Risk By signing this agreement, I acknowledge that: I understand and accept the risks associated with physical activity and exercise. I voluntarily assume full responsibility for any injury, illness, or harm that my child may sustain while participating in activities with LÊ TONE STUDIO. I certify that my child is physically fit to participate and has no medical conditions that would prevent safe participation. 3. Waiver & Release of Liability In consideration for my child’s participation, I hereby waive, release, and discharge LÊ TONE STUDIO, its owners, instructors, employees, agents, and representatives from any and all claims, demands, causes of action, or liability of any kind for injury, illness, or damages that may arise out of or in connection with my child’s participation, whether caused by negligence or otherwise, to the fullest extent permitted by California law. I understand that this waiver means I give up my right to sue LÊ TONE STUDIO or its owners for any injury or damages related to participation in these activities. 4. Indemnification I agree to indemnify and hold harmless LÊ TONE STUDIO and its owners from any claims, damages, costs, or expenses (including attorney’s fees) brought by or on behalf of my child, myself, or any third party related to participation in LÊ TONE STUDIO activities. 5. Emergency Authorization In the event of an emergency, I authorize LÊ TONE STUDIO staff to seek medical treatment for my child and agree that I am financially responsible for any medical services provided. 6. Governing Law This agreement shall be governed by and construed under the laws of the State of California. 7. Acknowledgment of Understanding I HAVE READ THIS CONSENT & LIABILITY WAIVER, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY. I UNDERSTAND THAT BY SIGNING THIS AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS.
